Verification
As part of studying the Doctor of Medicine you are required to attend Introduction to Clinical
Medicine (ICM) tutorials within local health services and clinical placements within general practice.
Students must be compliant with NSW Health vaccination and verification requirements before
attending any NSW Health site. If the below steps below are not completed students will not be
able to attend placement.
Step 1: NSW Health Compliance and Training Compulsory
Criminal Record Check
Vaccination Record Card - Page 2 contains instructions to assist you
Note: vaccination records and/or serology must be provided
COVID Vaccination Evidence of full course of COVID-19 vaccination (and booster when
eligible)
Attachment Form 6Undertaking / Declaration Form
Attachment Form 7Tuberculosis (TB) Assessment Tool
NSW Health Code of Conduct Agreement for Students
Attachment Form 9Hepatitis B Declaration Form
Blood Borne Virus Student Declaration Form
Influenza Vaccination (required to be updated every 12 months can be recorded on
vaccination card)

There are also a number of mandatory training modules that must be completed prior to attending an
NSW Health site via HETI My health Learning
These include:
Hazardous Manual Tasks and Work Health and Safety
Patient Privacy
Hand Hygiene
Between the Flags - Tier 1 (Awareness Charts and Escalation)
Cyber Security Fundamentals
Infection Prevention and Control Practices
Personal Protective Equipment for Combined Transmission-Based Precautions
Additional information on how and when to complete these modules will be provided to students.

Step 2: Charles Sturt University Requirements Compulsory
Working with Children Check (WWCC)
Please note: Students who are allocated to interstate placements, private hospitals or other private
organisations may also be required to complete site-specific requirements prior to commencing their
placement.
Step 3: Forward your Documents for Assessment
Please forward your documents through to FOS-WPL@csu.edu.au
with your details in the subject line
(example below).
These documents will be checked, logged in Inplace and sent to NSW Health for assessment and
verification. Please note assessment by NSW Health can take 2-3 weeks. Once verified by NSW Health
students will be notified of successful compliance.

All travel and accommodation arrangements, including costs associated with placement, are the
responsibility of the individual student.
