CONSENT FORM FOR MINORS TRAVELLING WITH A RESPONSIBLE ADULT
Unforseen matters may occur during a cruise which requires us to obtain specific consents from
the responsible adult travelling with your child (“the Guardian”). When you allow your child to
travel with someone other than their parent or legal guardian, you authorise the Guardian to
make decisions in the interests of your child, including all decisions which a parent or legal
guardian can make.
The Guardian may be asked to make decisions relating to matters such as your child’s safety,
health and dietary requirements, medical treatment and decisions relating to disciplinary
matters.
Please complete the authorisation below and provide a copy of this form at check-in
We recommend that the Guardian carry this completed consent form during their cruise.
