Sample Course Worksheet for LAC Application
For the LAC application, this is a general guideline for how the courses can be
listed. There is some flexibility with which courses you place in each slot. You
must list 15 AND ONLY 15 courses here. You must put at least one course in 8
AND ONLY 8 areas (you may list more than one course per section). Important
note: Do not list your Practicum or Internships (PC 595, 680) if you plan to use
those hours toward your licensure; they will not count if you list them here. The
following page is an example of a possible way to list your courses:
