1. Unofficial Transcripts
Please upload ALL copies of unofficial transcripts from every college, university, AP course, etc in the application, by
the Nursing Application Deadline. Most current Stan State students can skip this step. If you do not provide all
unofficial transcripts, you risk being disqualified for an incomplete application. This is one of the most common
reasons applications get disqualified. If Admissions have all final transcripts for courses taken at another
college/university, you do not need to submit any copies.
Be sure to double-check that your unofficial transcripts reflect what you input in this form. For example, if you had a C
in Chemistry and retook the course this semester and got an A, you may consider waiting for your unofficial transcript
to be updated with the new A.
Be sure to submit official transcripts (if applicable) to the University as well.

2. TEAS test
You must request official results be sent to us from ATItesting.com. Do not upload them to the application. This is one
of the most common reasons applications get disqualified, so be sure you order your test score through ATI and have
it sent to this university. If you take the test at Stanislaus State, the results are automatically sent to us. We will use
the highest score of your first 3 attempts. Sometimes it takes a few days for us to receive your score, so make sure to
plan to have your results sent so they arrive on time. You do not need to have your TEAS or application sent in any
order as long as both are received by the Nursing Application Deadline.
3. Additional and Optional forms
You can get extra points for including any of the first three items, but they are not required for a complete application.
• Healthcare Experience Form
• Language Proficiency Form
• DD214 or DD295 if active duty or veteran
• INS documentation if not a US citizen (or parents INS documentation if the student is under 19 years old)

4. Double-Check Your Eligibility
Please make sure you meet the minimum requirements below before proceeding.
• Overall GPA, Science Prerequisite GPA, and Non-Science Prerequisite GPA of 3.0 or higher.
• At least two science and two non-science perquisites completed with a C or better. (Prerequisites taken in Spring
2020 can be CR or Credit).
• All prerequisites completed or planned to be completed by the time the program starts with a C or better.
• TEAS score of 70 percent or better (total score only).
